Work Experience Agreement Form
The following 2012 Workplace Learning Agreement Form MUST be completed by both parents / guardians AND the Employer and returned to school PRIORto going on Work Experience. This form is returned to your PLP Teacher.
Section 1: This Work Experience Journal is to be completed while you are on Work Experience.
Interacting with others to identify and refine your goals There are components in the Work Experience Journal which require you to interact with your employer and ask them questions.
Your Employer will be assessing you on your initiative and confidence in discussing your goals with them.
The performance standard being assessed here is:
A Interacts purposefully with others to identify and refine goals.
B Generally interacts effectively with others to identify and refine goals.
C Interacts with others to identify and make some refinement to goals.
D Interacts with others with limited effectiveness, to talk about possible goals.
E Gives limited responses to questions from others about possible goals.
Section 2: When your return to school the Reflection Tasks will be completed in class. One week will be allocated to completing the Reflection Task DUE DATE: END OF WEEK 5, TERM 2
Developing the Work Capability
When you go out on Work Experience there are some important things to understand.
It is essential that you have completed the Occupational Health, Safety and Welfare task to ensure your wellbeing and safety whilst on Work Experience.
